OV codes showing in your forms normally mean that the form hasn't been saved correctly. Please follow the below steps to resolve this issue:
Re-run the template from the workflow.
When the document is open, go to File and then Save.
Close the document.
Press save in DPS.
If this issue still persists after following the above steps then please ask a question in the MyAccess Portal Digital Assistant and ask to "Speak to a human" to be put through directly to our support team.
